What would you say if someone in your family asked what you would do if they let their boyfriend move in with them? Specifically, staying over night at her house when you go to visit.
I would say no I cannot stay over at your house any more.
Would you ever step foot in her house with a live in boyfriend, though? Or would you avoid the place all together?

Consider the reality of many non-Catholic marriages today: the couple intended to never have children, thus intended not to contract marriage. Thus they are also cohabiting. It doesn't make sense to treat this situation any differently than the one you describe.

I would not go to a person's house if they were living with their boyfriend. It may be a big pain at times but that would be the way it is.
As far as other situations where you are unsure if they are truly married, I think you err on the side of "yes, they are married." I believe matrimony is the only sacrament where you err in the direction of "yes, valid" but that is just my thoughts.
